Product Evaluation Report Form Info

A Product Evaluation Report (PER) is a document issued by the Export Coordination Division (ECD) or Export Division (ED) of the Bureau of Customs (BOC) after pre-evaluating export products applying for a Certificate of Origin (CO) and ensuring compliance with BOC requirements.
The Product Evaluation Report (PER) shall be valid for a period of five (5) years from the date of issuance.
Notwithstanding its validity, the exporter must apply for a new PER for the same product in cases where tariff rate of an export product is affected by the issuance of a new ASEAN Harmonized Tariff Nomenclature (AHTN).

List of unfinished substances or unrefined natural resources used to manufacture finished goods that comes from other country
Local
List of unfinished substances or unrefined natural resources used to manufacture finished goods that already available locally
Country of Origin
Name of the county where the raw materials originated
CO Form
Refers to the international trade document attesting that goods in a particular export shipment were wholly obtained, produced, manufactured, or processed in a particular country.
